General Organization & Philosophy

Our mission statement is “Creating Opportunities for and with People.” We aim to create meaningful connections and provide specialized support for individuals so they can live a life of choice and opportunity.

At RISE, we don’t believe in a “one-size-fits-all” approach. We work collaboratively with the individual, their family, and their support team to follow an Individual Support Plan (ISP). This ensures that every service—whether in-home, residential, or employment-based—is tailored to the specific preferences and goals of the person we serve.

We have a broad presence across the state, with local offices and programs in major hubs including Albany, Bend, Eugene, La Grande, Pendleton, Roseburg, Salem, and surrounding communities. This allows us to provide localized, accessible care to families throughout Oregon.

Funding & Eligibility

RISE provides a variety of supports for children, transition-age youth, and adults with intellectual and developmental disabilities. Eligibility is typically determined through the Office of Developmental Disability Services (ODDS). We recommend contacting your local Community Developmental Disabilities Program (CDDP) to discuss your specific needs and referral options.

Most of our services are funded through Medicaid waivers administered by the State of Oregon. We also work with families on private pay options or specific county-funded programs, depending on the service area and availability.

Program & Support Details

DSPs are the heart of our organization. They provide one-on-one support ranging from daily life skills (cooking, cleaning, and medication assistance) to community outings and advocacy. In Oregon, our DSPs undergo rigorous training, including Oregon Intervention Systems (OIS) and CPR/First Aid, to ensure the highest standard of safety and care.

Yes, transportation is a core component of many of our services. Whether it is getting to a job site through Employment Services, attending a community event via In-Home support, or traveling to medical appointments from a Residential home, we aim to remove barriers to community access.

RISE provides a continuum of care that spans almost every stage of life. We offer programs for children and youth, as well as residential, supported living, employment, and day services for adults.

We maintain strict compliance with Oregon state licensing and health standards. Our staff undergoes rigorous background checks and continuous training.

The primary difference lies in the level of independence and the setting. Residential Homes provide 24/7, around-the-clock care for 2 to 4 individuals in a shared home owned or managed by RISE. This model is ideal for those requiring intensive, constant support and supervision. Supported Living, on the other hand, is designed for individuals who live in their own homes or apartments (either alone or with a roommate). Support is flexible and offers “wraparound,” meaning staff visit at scheduled times to help with specific tasks like budgeting, grocery shopping, or medical appointments. It is a highly autonomous model focused on maintaining a person’s independence in their own private residence.

Yes. We provide comprehensive initial and ongoing training for all staff and providers. This ensures that our team is equipped with the latest best practices in the human services field and can provide the highest quality of care.

Our Employment Services team works with local Oregon businesses to match them with motivated, pre-screened candidates. We provide job coaching and support at no cost to the employer to ensure a successful, inclusive workplace partnership.
